Take I-680 North toward SACRAMENTO. Take the TREAT BLVD/Geary Road exit. Make a right onto TREAT BLVD. Make a U-TURN at CHERRY LN onto TREAT BLVD westbound. Right turn into 1411 Treat Blvd, right before the BART station.

From North of Walnut Creek (Benicia, Martinez, etc.)
Take I-680 South toward MARTINEZ/SAN JOSE. Take the exit toward TREAT BOULEVARD/GEARY ROAD.
 Turn LEFT onto N MAIN ST. Turn first LEFT onto TREAT BLVD. Make a U-TURN at CHERRY LN onto TREAT BLVD. Right turn into 1411 Treat Blvd, right before the BART station.

From West of Walnut Creek (Orinda, SF)
From Hwy 24 take I-680 N toward SACRAMENTO. Take the TREAT BLVD/Geary Road exit. Make a right onto TREAT BLVD. Make a U-TURN at CHERRY LN onto TREAT BLVD westbound. Right turn into 1411 Treat Blvd, right before the BART station.

Referral Form
Patients will be evaluated by a licensed veterinarian. We ask that surgical or neurological patients be accompanied by referral information and include any pertinent records and radiographs. We evaluate chronic osteoarthritis, soft tissue

Client Forms:

injuries, weight loss and/or conditioning patients to determine if our program can benefit them. We
request that patients have proof of current vaccinations. Careful records will be maintained which will
include initial assessment measurements, response to treatment modalities and an outcome assessment
for each patient. Referring veterinarians will receive progress reports on their patients.

Patient Intake Form
Please complete the patient intake form for your pet prior to your first appointment. Bring a copy with you
to your initial exam. Please plan to arrive 15 minutes prior to your appointment time to allow plenty of time
to complete the intake form.
