Referral Information

When making your first appointment, we will work with you to obtain the appropriate referral information and medical records from your regular veterinarian as well as any other veterinarians who have been involved in your pet's medical care. This includes referral information and any pertinent records and radiographs. We also ask that patients have proof of current vaccinations including rabies. At the first consultation, your pet will be examined by a licensed veterinarian. We will evaluate your pet’s chronic osteoarthritis, soft tissue injury, weight, conditioning and other aspects of his or her health to determine if our program can benefit him/her. We maintain meticulous records which will include response to treatment modalities and an outcome assessment for each patient. Referring veterinarians will receive an initial evaluation report.

Patient Intake Form

We will email you a client and patient registration package prior to your first appointment.  We ask that you complete them and return them to us prior to your initial consultation. Please plan to arrive 15 minutes prior to your appointment time to allow plenty of time for the registration process and maximize time in the consultation with Dr. Troy.
